Does EquiLife require specific insurance coverage documentation to be authorized to conduct the lab test review with your clients?Updated 7 months ago

Insurance coverage documentation is required for some practitioners, determined by the level of practitioner licensure. 

The following practitioners do NOT need to provide insurance documentation: 

- Chiropractor (DC) 

- Medical Doctor (MD) 

- Naturopathic Doctor (ND) 

- Osteopathic Doctor (DO) 

- Nurse Practitioner (ARNP) 

- Physicians Assistant (PA) 

- Nurse Midwives (CNM) 

- Doctor of Medicine in Dentistry or Doctor of Dental Surgery (DMD/DDS) 

- Dr. of Oriental Medicine (DOM) 

- Dr. of Traditional Chinese Medicine (TCM)


The following practitioners/providers are authorized to provide their clients’ their lab test reviews by providing the following insurance coverage documentation: 

- Integrative Health Practitioner 

- Level 2 Certified (IHP2) 

- FMCA Health Coach (IFM) 

- Functional Diagnostic Nutrition Coach (FDN) 

- Certified Nutritionists (CCN) 

- Functional Nutritional Therapy Practitioner (FNTP) 

- Licensed Acupuncturist (Lac) 

- Registered Dietician (RD)


$3 Million aggregate/$1 Million per claim in Liability coverage, practicing as a health coach and also covering lab education.
